  1. Check to see if you are at risk of type 2 diabetes by visiting – https://riskscore.diabetes.org.uk/start
  2. If you have a risk score of 16 or over contact your GP to arrange a blood test, this will confirm if you are at risk of diabetes
  3. If your blood test results show you are at risk of type 2 diabetes you can complete our self-registration form
  4. Once your registration form has been submitted, the team will contact you to start the programme and book in your first session

We’re all unique and everybody will have a different amount of weight they’re both able to, and need to, lose, to improve their health and help reduce their risk of developing type 2 diabetes or manage their current condition. As a rough guide, we suggest aiming for losing 5% of your starting weight for those who would benefit from weight loss. This will have positive health benefits some of which you will hear about in the sessions.

Weight is a sensitive issue for many of us and getting to an ideal, healthy weight is easier said than done. Rather than focussing on a final goal weight, which can feel too far away, try setting yourself smaller, more achievable changes. Ones that feel sustainable and will help you to feel confident that you are doing your best to lose weight by making healthy choices and not purely basing success on what the scales are saying.   Our programme will help you develop a healthy relationship with food that will also help you manage your weight for the long term and reduce your risk of developing type 2 diabetes.

Absolutely! You won’t actually be taking part in any exercise within the sessions. If you attend one of our virtual sessions, you’ll be able to do so from the comfort of your own home and our face-to-face groups will always have seats for your comfort.  Our coaches will do their best to make sure you are able to access the sessions.  Please discuss any individual needs you may have during your discovery call. 

To be eligible for our programmes you need to be at risk of developing type 2 diabetes which is sometimes called pre-diabetic. To find this out you can: 

  1. Check to see if you are at risk of type 2 diabetes by visiting – https://riskscore.diabetes.org.uk/start
  2. Ask your GP for a blood test to check if you are at risk of type 2 diabetes. If your blood test results show a fasting blood glucose reading between 5.6-6.9mmol/L or HbA1c 42-47.9mmol/mol you are eligible for our programme and can complete our registration form

Of course! We fully appreciate that life can be busy, and work can often make things tricky especially shift work patterns.  Attending F2F sessions, you will know in advance the dates times and location of your group, helping you plan and know how many sessions you can attend over the 9 months.  If you feel you would be missing more than 4 of the 13 sessions let your Healthier You Coach know as soon as possible, you may be able to catch the session up with another group at a better time.  Alternatively we have a digital option that offers flexibility that may suit you better.  As a service we will try our best to enable your successful attendance and access to this program.

We’ll give you a journal where you can keep track of your progress on a weekly basis. You’ll find a weight and waist tracker in it as well as areas to track other valuable aspects of your health, like your mood, emotions, sleep and movement. It would be great if you can share with your practitioner a regular weight (self-reported if you’re doing our digital programme, or taken in person if you’re doing our face-to-face programme). We love to hear how your week has gone so we can give you the support you need to achieve your goals.  

Once you’ve completed our Healthier You programme if you feel you need a little extra support, your practitioner can discuss other local options that may be able to help you continue to make and sustain changes.  You can access all materials and resources sent out during the course as they are yours to keep.
