Our dedicated team will help your patients reduce their risk of type 2 diabetes using a clinically-proven intervention as well as motivational support and encouragement every step of the way. We start with a short initial assessment (by phone or online) to help determine which of our NDPP programmes will best suit their needs and lifestyle.
You can easily refer using your clinical operating system e.g., Systm1 or EMIS web. The referral goes straight to our secure nhs.net inbox and we will contact your client to triage.
If a patient has ‘non-diabetic hyperglycaemia’ (NDH) identified by blood test within the last 12 months. They can complete a self-registration form and sign up for the programme.
Please Note:
In order to complete this form they will need the following information provided to them by a GP or HCP.
This may be a useful option for practices that do not have the capacity to process referrals themselves but are able to provide patients with the clinical information needed so they can self-register.
The NHS DPP self-referral option was removed on the 15/11/2022. Self-registration is a replacement for this referral route.
Non-Diabetic Hyperglycaemia (NDH)
If the patient has a history of Gestational Diabetes Mellitus (GDM) then no blood test is required.
