Healthier You Hypertension Programme

This programme is for people who have been recommended NHS support by their GP because they have high blood pressure (hypertension) and want to improve their long-term health.

High blood pressure can increase your risk of future health problems such as stroke, heart disease, kidney disease and dementia. The good news? Making small lifestyle changes now can help protect your long-term health and the NHS is here to support you.

The Healthier You: Long Term Conditions Prevention Programme is a friendly NHS programme designed to help you build healthier habits and reduce your risk of developing serious health conditions in the future.

How the programme works

The programme gives you personalised support to help you:

  • eat more healthily
  • move more
  • reach or maintain a healthy weight
  • understand your risk and take practical steps to improve your health

You can choose how you take part:

Group

Our face-to-face group programme is run at a range of local venues. Meet like-minded people and attend group sessions each fortnight to learn about healthy living and share your experiences.

Digital

If attending regular in-person sessions isn’t right for you because of work, family or health commitments, a digital option is available.

Get free access to the Second Nature app where you receive personalised support from your Health Coach, access to in-app features and hundreds of delicious recipes.

What happens when you sign up?

Once you register:

  • One of our friendly team will contact you by phone or email
  • You’ll have a short 1-to-1 session so we can learn more about your lifestyle, health, goals and anything that might make change harder for you.
  • You’ll be offered dates, times and locations for face-to-face sessions or
    you’ll receive login details if you choose the digital option
